To all attendees bringing a minor to any NCCC event:
Parents/guardians and the minor may sign the waiver in the link below at the event. However if a non-custodial person (relative/neighbor) is bringing a minor to the Toy Run (or any other NCCC event), the parent must sign this form in advance AND attach a photocopy of their government issued photo ID to the form. The Host club is required to keep these minor waivers (with ID when relevant) until the minor participant turns 18.
Here is a link to the Minor Waiver for your use if you will be bringing a minor for whom you are not the legal guardian. Please note the waiver is two pages, one for the adults and one for the minor.
